Follow the below easy steps to request testing from your healthcare provider and with your insurance.
Download, print, and complete the patient portion of the laboratory requisition (pages 3-4 in the discussion guide).
Take the completed discussion guide in with you to your next appointment.
Speak with your provider about the testing and ask your provider to complete the provider section of the laboratory requisition form (page 3 of the discussion guide).
Email the completed requisition (page 3 of the discussion guide) into orders@allergenis.com

If you and your healthcare provider agree that the Allergenis testing is right for you, your healthcare provider completes the provider portion of the requisition. The blood draw may happen in your providers’ office or through a mobile phlebotomist which comes to a location and at time convenient to you.
